Communication with Referring Physicians and Implementation of Telemedicine Services

Increasingly today, hospitals deplore a shortage of qualified medical specialists. It is therefore obvious that these physicians are not available in all the locations where they are required. For the purpose of ensuring, nevertheless, top-quality patient care, interest focuses increasingly on telemedicine applications. It is through the support of modern technology that each hospital can access expert know-how – at a global level. In addition, this approach helps in establishing cross-sector and integrated models of care. Rapid and seamless communication of image and reporting information is an important cornerstone of structures such as these.

Efficient Communication Among Healthcare Providers

VISUS offers a multitude of options to hospitals with the aim of installing efficient communication between various healthcare providers in the care chain. The technical implementation of connections such as these is determined by individual targets and project-oriented requirements.

The synchronous point-to-point link is, in view of the multitude of connection models, still the “classic” concept for teleradiology: two DICOM nodes are connected, and communication takes place via secured connections in a VPN tunnel (Virtual Private Network). With large bandwidth available – e.g., DSL -, links with the performance and security required can be set up for data transmission.

DICOM e-mail is another option for communication which has been proven and tested in routine work. It can be integrated easily into the existing IT infrastructure and allows for the implementation of a global, vendor-independent telemedicine network. By use of the ubiquitous e-mail technology and by integration of existing infrastructures, connections with referring physicians and further actors in healthcare can be created very easily. The communication and image viewing modules can be integrated easily into each PACS. This enables highly efficient image communication via secured data connections for applications which are not time critical.

Exchange of information via secured e-mail allows providers of care to participate in telemedicine networks at no great technical effort or financial expense – because an ADSL connection is sufficient for the receipt of image data. The benefit for care providers, as well as for their patients is in reduced time and added efficiency. DICOM Mail is particularly suitable for connection with referring physicians and for communication with an open group of participants.

The system is integrated seamlessly into the image workflow of a department. Each authorized user can initiate, at the click of a mouse, dispatch via e-mail of entire studies or of single images. In doing so, common image compression techniques are applied. Incoming DICOM e-mails can be imported easily into the patient context of the image database.

Workflow Optimization in Solutions for Groups

Groups of care providers have additional requirements regarding telemedicine solutions. In order to cope with them, VISUS has integrated a resource planner for on-call duty into its system. This ensures that incoming images are passed on automatically to the physician on duty. This intelligent routing function safeguards a smooth workflow and rapid, target-oriented transmission. It is in particular in emergencies that this feature is of key importance. In turn, reporting texts are sent back automatically to the senders of the images.

Use of Teleradiology to Obtain an Expert Opinion

Besides obtaining – on a per-case basis – a second or expert opinion, a physician’s background service from home can be realized using JiveX. This requires a suitable image viewing device at the site of the expert as well as a secure data connection between the actors.
